Care Companion
Do you enjoy living independently, but need some assistance from time to time? Or do you have a loved one who has some health challenges but wishes to remain in the comfort of their own home? Our program called Care Companion may be just what you need! Care Companion is a video telephone that is connected through a standard telephone line to a nurse who can see and talk to you while taking your vital health measurements. With the simple touch of a button, our nurse will monitor your blood pressure, blood sugar, weight, blood oxygen, and other vital statistics. Our program also offers a “Family Connection” package. Now family members living far away can have the opportunity to talk and see their loved one during phone visits. Think of the peace of mind you and your family will have with Care Companion. Independence, Peace of Mind and Quality of Life are important to all of us during our life’s journey. Lutheran Homes and Health Services is here to offer another option in our continuum of care. Respite Care
The Lutheran Home also offers both short and long term respite care. Respite care is designed for the caregiver who needs to take some time for him or herself. We offer respite stays from one day to one month! Many members of the Fond du Lac community have taken advantage of this service, and have been able to do so knowing that their loved one is in excellent, caring hands.

Short Term Rehabilitative Stays
With our recent Medicare certification, we have been able to offer a broad range of short-term, rehabilitative stays for those who have suffered a physical setback, such as a fracture or a stroke. Our interdisciplinary team works together to help individuals regain their strength, with the goal of getting the person back home or to the most independent setting available. We have touched the lives of many young/older adults with this program, and are excited to be part of helping people to "feel like themselves" again!

Wellness and Rehab Center
The Wellness and Rehab Center is an outreach of the Fond du Lac Lutheran Home, Inc. We provide Physical, Occupational and Speech Therapy to inpatients of the facility as well as to outpatients from the greater Fond du Lac community.
