Strategic Plan Update
December, 2004
Lutheran Homes & Health Services has long been known for its proactive and forward-thinking approach. Over the past year, we have embarked upon and completed initial phases of our Strategic Plan that will serve as our guide for growth and development over the next five to ten years.
We are excited to share with you some of the details of that plan, which will serve as our guide for organizational growth and development in the near future.
Some phases of the plan are already underway:
- Post-Acute Services
Over the past three years, we have seen a significant increase in our return to home rates. Specifically, 43% of patients who choose the Lutheran Home for their short-term rehabilitation successfully return home. We are proud of our expertise in this area, and recognize the need to refine that expertise by providing a separate area within the building that is tailored specifically to meet the needs of short-term rehabilitation patients. Our post-acute unit will open its doors on December 7, 2004 and will feature amenities that will make a short-term stay more comfortable, and as home-like as possible. Those who stay with us for their short-term needs will be provided expert rehabilitation and clinical services that are second to none.
- The Wellness & Rehab Center – Program Expansion
The wide array of programming available through the Wellness & Rehab Center continues to flourish. As we become known not only for our expertise in physical, occupational and speech therapies, but also for our specialty clinics, the demand for more space has increased. The Wellness & Rehab Center has been expanded on the lower level of Tarfa Terrace, to include additional treatment and consultation rooms, and allow separate treatment areas for inpatients and outpatients, recognizing the different needs and goals of these two segments of the population.
The strategic document emphasizes long-term growth, including a new campus that will be home to additional senior housing, assisted living and various community programs. We encourage your feedback and participation, whether you are an old friend or just visiting our website for the first time. We know it is through those we serve that we learn.
