Testing the Effectiveness of Telemonitoring (Pilot Project)

Major Research Findings:
Subjectively - high user satisfaction which translated into increased awareness of their health conditions and early detection of health changes. Objectively -- inconclusive data to determine decreased use of ER visits or unscheduled monthly visits (translating into cost savings). Larger sample size and time frame needed.

Lessons Learned

We have seen first hand that telemonitoring clients take more ownership for their health status and simultaneously are more compliant with taking medications, following diet, and doing what they can to prevent health issues. Additionally, trusting client/nurse relationships have been developed through the telemonitoring experience, effecting overall well-being for the client, including decreased anxiety levels and enhanced self-esteem.

We have learned that the best application for telemonitoring is in the home setting. During our pilot, we conducted testing in an assisted living setting as well as independent setting for seniors. Throughout the process, we did find duplication of services in some cases. Our Director of Marketing addressed the issue of telemonitoring in assisted living environments at the 2004 Healthcare Unbound conference in Boston.

We learned that telemonitoring did help us to intervene in order to avoid an acute episode in several cases. In one instance, a woman who had four hospitalizations the year prior has not had a single hospital stay since becoming a care companion client over one year ago.

We have learned that a person with memory impairment was able to successfully operate the equipment. Given the face-to-face interaction through the video monitor, our nurse is able to step the client through use of the peripherals for monitoring vitals.

Next Steps:
Lutheran Homes & Health Services, Inc. has received a grant from the National Equity Fund to provide telemonitoring to senior residents in affordable housing environments. This will be accomplished through the use of a multi-user telemonitoring system. In addition, we continue to work with IFAS (the Institute for Future Aging Services) through the American Association of Homes and Services for the Aging.
